Community of Hope Health Services strives to improve health outcomes by providing quality, affordable, and accesible primary healthcare with a focus on serving the uninsured and underinsured in our community. We have three locations across the city: Marie Reed Health Center in the Adams Morgan neighborhood, Family Health and Birth Center in Ward 5, and Covenant Health Center in Ward 8.
Our goal is to provide a Medical Home for patients, where they have a trusting relationship with a healthcare provider who knows them, and they can receive the care they want, when they need it. No one is turned away based on inability to pay. Most insurances are taken, and a sliding fee scale is available.
About 210,000 residents in DC are medically underserved; most live in the city’s Ward 8. With the advent of healthcare reform, they will all have insurance, but will still need compassionate, accessible providers. Community of Hope is in the planning stages for a health and resource center in Ward 8, with a projected completion date of 2013.)
Primary Medical Care: Services include comprehensive preventive care and disease management for pediatric, adult, and prenatal patients. As a family medicine practice, we integrate the physical, social, and spiritual needs of each patient within a family context, with an emphasis on health education and personal well-being.
Dental Care: Services include comprehensive oral examinations, X-rays, dental cleaning, scaling and root planing, fluoride treatment, sealants, restorative work, extractions and dentures.
Mental Health Services: We have on-site social workers who provide therapy and assessment, and a part-time psychiatrist who provides medication management. Medical case managers assist patients with HIV/AIDS, though services for any patient in need are available.
Youth/Young Adult Services: The young adult clinic is a confidential primary healthcare service for DC residents ages 13-24. It is held every Tuesday from 3:00 pm – 7:00 pm. Walk-ins and appointments are available. Please bring a picture ID (school ID is ok). Services include: general medical care, pregnancy testing, counseling, and prenatal care, STD and HIV testing, contraception, school and sports physicals, and health classes and discussions.
Electronic Health Records (EHR): In July 2010, Community of Hope Health Services transitioned to Electronic Health Records (EHR). All patient records are now kept electronically. This change allows us to give better care. We use eClincalWorks, which is used by many health centers in DC. As required by law, health information remains confidential, seen only by necessary staff.
Electronic health records lets our providers easily see their patients’ health information and know how best to treat them. Prescriptions can be sent electronically to pharmacies. Test results go directly to patients’ medical records. And specialists get the information they need quickly.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
